Link Life Fife - Supporting Mental Health and Long-Term Health Conditions

Link Life Fife is a community support service for adults (18+) living in Fife who are experiencing mental health difficulties or managing a long-term health condition that affects their wellbeing.

We provide practical and emotional support to help you cope with challenges such as stress, anxiety, low mood, or feeling overwhelmed, and to improve your overall quality of life.

How will Link Life Fife support you?

Our Link Life Fife worker will:

  • Listen to what matters most to you
  • Identify your needs, skills, and strengths
  • Provide information and options to help you make informed decisions
  • Support you to access health services, community resources, and specialist support
  • Connect you with activities or social groups to reduce isolation
  • Help you make positive changes to improve your wellbeing

Who can access the service?

Link Life Fife supports adults aged 18 and over who are experiencing any of the following:

  • Persistent poor mental wellbeing
  • A long-term health condition that impacts mental health or daily life
  • Emotional distress linked to social circumstances

Special Announcement

Link Life Fife is enhancing its service to better support people living with long term health conditions, thanks to funding from the Primary Care Improvement Fund. While the service continues to focus on mental health and wellbeing, it now includes dedicated support for those managing conditions such as diabetes, heart disease and chronic pain.

According to NHS Scotland, people living with chronic pain and long term conditions often experience low mood, anxiety and depression, and benefit from the same types of non-medical support as those with mental health challenges.

To meet this growing need, new link workers have joined the already established team which supported 1658 people in 2025. Referrals can be made from Primary Care Professionals from across Fife.

How can I make a referral to the service?

Referrals are accepted from health professionals in Primary Care from GP practices across Fife.

Speak to your GP, nurse or other health professional, who can refer you to Link Life Fife.
