Community Cardiac Specialist Nurse service
Heart failure is a syndrome consisting of symptoms such as breathlessness, fluid retention & fatigue. These symptoms occur when the heart is struggling to pump or relax as it should to ensure enough blood, oxygen & nutrients are circulated around your body.
Heart failure is a lifelong chronic condition requiring medication and sometimes other treatments. Palliative care needs may be considered alongside this and can when appropriate include end of life care.

Community Diabetes Specialist Nurse service
We are a small team of Community Diabetes Specialist (CDSNs) who are all prescribers. Our service acts as a link between your GP practice and the hospital diabetes service for people living with complex type 2 diabetes (already on or requiring to be assessed for insulin). We aim to support people to self-manage their diabetes, reduce or prevent the complications of diabetes, prevent hospital admission or readmission and enable people living with diabetes to live long healthy lives.
