A Fife Health and Social Care Partnership service has been enhanced to provide support to people living with long term health conditions.
Link Life Fife is a community support service for adults living in Fife.
Link Workers help people who are referred into the service with non medical concerns that may be affecting their health and wellbeing such as finance, housing, loneliness or access to community resources.
The service has a focus on providing support to people with mental health and wellbeing difficulties and engaged with 1658 people in 2025.
Recently, the service was extended to provide support to adults managing long term health conditions and two new Link Workers have joined the team to help support more people in the Kingdom.
Jacquie Stringer, Service Manager, Community Led Support, said: “We are now able to provide dedicated support to people managing conditions such as diabetes, heart disease and chronic obstructive pulmonary disease who find their condition is affecting their wellbeing.
“Often people living with long term conditions experience stress, anxiety, low mood or feel overwhelmed. We can provide practical and emotional support to help people cope and improve their overall quality of life.
“We hope by extending our service we can help more people address the concerns they face and are delighted to welcome our new Link Workers Annie and Alice to the team to help us do that.”
A Link Worker will:
- Listen to what matters to the person,
- Identify their needs, skills, strengths and priorities,
- Provide information and options to help them make informed decisions,
- Support them to access community resources, and specialist support,
- Help them with non medical issues such as financial worries, housing challenges or social isolation,
- Connect them with activities or social groups to reduce loneliness,
- Help them make positive changes to improve their overall wellbeing.
People can access the Link Life Fife service through a referral from a Primary Care professional, such as a GP, Mental health Nurse or Allied Health Professionals.
