Improving Support For Moving From Children's Services to Adult's Services - Young People

We are improving the way we support young people with additional or complex needs as they move from children’s services to adult services.
These improvements are designed to make planning for the move earlier, clearer and less stressful for young people and their families.

We want to hear from young people who have experience of moving from children’s services to adult services. What you tell us will help to make sure the improvements we are making are the right ones.
