The North East Fife “Creating Healthy Communities” Roadshow Workshop was held in May 2017. Following on from that, those who attended the Roadshow and other key staff were invited along to Locality Planning ‘Wider Stakeholder Group’ meetings in December 2017 and April 2018, to identify, discuss and agree the emerging North East Fife Health and Social Care priority areas for action.

Three priority models have been agreed:

  • A community led one point of access concept ‘The Well’, with the support of National Development Team for Inclusion (NDTi) Community Led Support Programme.
  • Locality Model of Care Providers pilot – in-house and external partnership funded care provision working closer together to enable people to remain in their own homes and communities.
  • Link with St Andrews ‘Hub’ to explore joined up ways of working (with a focus on social isolation).

The next steps will be to collate the areas for action into an Agreed Priorities Table for NEF and to establish the Health and Social Care Locality Planning ‘Core Group’.
